Choose a location from this this list of approved mechanics. When you call to make an appointment, tell them you need an inspection for HopSkipDrive.
- Print a copy of this form and bring it with you to the inspection.

The mechanic will fill out the form when they inspect your car and will upload the information to the City of Seattle Regulatory Compliance and Consumer Protection site. Once your inspection is complete and the information has been uploaded, check your email for a City of Seattle PDF like the one shown below.
Upload a copy of the City of Seattle PDF using the CareDriver app. If necessary, HopSkipDrive can also accept a picture of the paper inspection form, but the City of Seattle PDF is preferred.
