California: Medical Exam, TB Exam, and Drug Test

Safety is our top priority. To get you ready for the road, all CareDrivers in California must complete a Tuberculosis (TB) assessment, a medical exam, and a drug test.

These instructions apply to hopeful CareDrivers applying to drive with HopSkipDrive. If you are already a CareDriver renewing your documentation, follow the instructions in this article.

How to schedule a medical exam, TB assessment, and drug test

Follow the steps below to schedule the required testing:

  1. Schedule an appointment with Carbon Health here.  
     
  2. Under What brings you in? select:
    •  DOT Physical if you need a medical exam, a TB assessment, and a drug test. 
    • TB Testing if you only need a TB assessment and a drug test. 
       

  3. Print this packet containing your authorization form, TB Risk Assessment form, and medical exam form. On the authorization form, print your name, date of birth, and the date at the top. 

    Under Services Requested: 
     

    • Select 5-Panel Drug Test (with THC). This is required for everyone. Next, select the options that apply:
      • If you need a medical exam and a TB test: select DOT physical and TB Assessment.
      • If you only need a TB test: select TB Assessment.

      • If you only need a medical exam: select DOT physical.

  4. Bring the following to your appointment:
    • Your full printed authorization packet.
    • Your valid driver's license
Note: You must fill out the authorization form before your appointment. If you do not have your name, date of birth, the date, and the services required filled out, you may be turned away and asked to return at a later date. Please follow the instructions outlined above to prevent rescheduling.

Already have recent results?

We can accept your TB clearance and/or DOT medical exam if it was taken within the last 2 years. Please visit your application portal to upload your documentation.

Please note that a new drug test at Carbon Health is still required for all applicants.
