To ensure the safety of our riders, all CareDrivers in Arizona must complete a Child Abuse and Neglect Screening (CANS) through the central registry with the Arizona Department of Child Safety (DCS). You'll need to request your records by completing form DCS-1083A. Results will be sent directly to HopSkipDrive.
Before you start, have your IVP fingerprint clearance card — or your application, if your card is pending — available. You'll be required to enter your clearance card or application number.
Follow the steps below to complete your CANS requirement in Arizona:
- Click this link to access the Direct Service Central Registry Clearance Form (DCS-1083A).
- Complete all fields, including your personal information, IVP card or application number, and address.
- Sign the form and enter the current date.
- Click Submit at the bottom of the page. Following your submission, HopSkipDrive will automatically email the completed form to DCS.
Results from the background check will be emailed directly to HopSkipDrive within 2–3 business days. Processing times may vary.
