California CareDrivers: Medical and TB Exams

CareDrivers California must pass a Tuberculosis (TB) assessment and medical exam. Both TB assessments and medical exams are valid for 2 years from their completion date (at the medical provider's discretion).

How to schedule a medical and TB exam

If you are in need of a TB assessment and medical exam, please follow the steps below:

Note: CareDrivers in Bakersfield, see below for specific instructions.


  1. Schedule an appointment with Carbon Health here. 

    • In Location, select the location nearest me to view clinics available in your area. 

    • To help the clinic allocate the appropriate amount of time for your appointment, under What brings you in? select DOT Physical if you need a medical exam and TB assessment OR select TB Testing if you only need a TB assessment. 

  2. Print this packet containing your authorization form, TB Risk Assessment form, and medical exam form. On the authorization form, print your name, date of birth, and the date at the top. Learn more about completing the authorization form here.

  3. Under Services Requested, select the services you need (refer back to your applicant portal if you are unsure). Carbon Health has the ability to conduct both a TB Risk Assessment* and DOT Physical as needed. 

    *TB Risk Assessment should be completed FIRST. As determined by the medical provider, it may be recommended that you complete additional testing. HopSkipDrive will cover the cost of a TB skin test, while the expenses for a blood test or chest x-ray will be your responsibility.

    Bring the entire packet and your driver's license to the exam.  

       Note: You must fill out the authorization form before your appointment. If you do not have your name, date of birth, the date, and the services required filled out, you may be turned away and asked to return at a later date. Please follow the instructions outlined above to prevent rescheduling.

CareDrivers in Bakersfield

Carbon Health locations are not available near Bakersfield, so HopSkipDrive has partnered with a partner called Mobile Health. Follow the instructions below to obtain a medical exam or TB assessment in Bakersfield.

  1. Log into the Mobile Health portal using your personal email and the authorization code: HSDDOT (Mobile Health will email you another 6 digit code to continue scheduling).

  2. In Organization, select HopSkipDrive- Bakersfield CA. Click Schedule now and enter the required demographic information.

  3. Select the services you need. You can choose:

    • TB risk assessment*
    • DOT physical
    • Drug testing

      If you have a valid TB clearance or DOT medical exam taken within the last 2 years, please upload documentation via your application portal. 

  4. Select a desired appointment date (at least 48 hours in advance) and time at our partnered location:

  5. Confirm your appointment and check for a confirmation email.

  6. Print this packet containing your TB Risk Assessment form and medical exam form and be ready to present the packet, your confirmation email, and your driver’s license at the appointment time.

TB blood test or chest x-ray for Bakersfield CareDrivers

A TB Risk Assessment should be completed first, following the instructions above. Based on the risk assessment, the medical provider may recommend additional testing. HopSkipDrive covers the cost of the initial TB Risk Assessment, but the expenses for a blood test or chest x-ray will be your responsibility. If it is determined that you need additional TB testing, follow the instructions below to schedule an additional appointment for testing through Mobile health. 

  1. Log in to the Mobile Health portal here using your email and the authorization code HSDQFT (note that this is a different authorization code that you used for the initial appointment). Mobile Health will email you another 6-digit code to continue scheduling.

  2. In Organization, select HopSkipDrive- Bakersfield CA. Click Schedule now and enter the required demographic information.

  3. Select the required services:
    • Quantiferon Blood Test (must be completed first) 
    • Chest X-ray (only required if blood test is positive)

  4. Select a desired appointment date (at least 48 hours in advance) and time at our partnered location:

  5. Confirm your appointment and check for an email confirmation.

  6. Be ready to present your confirmation email and your driver’s license at the appointment time.

Already passed a TB clearance or DOT medical exam?

We can accept your TB clearance and/or DOT medical exam if it was taken within the last 2 years. Please visit your application portal to upload your documentation.
