All Colorado CareDrivers must pass a Transportation Network Company (TNC) medical exam.
If you have passed a TNC medical exam within the past 18 months, there is no need to have another exam. Submit a copy of your current medical exam certificate via the CareDriver app.

TNC Medical Exam
- Make an appointment with the physician of your choice.
- Ask your provider to complete this TNC Medical Exam form. You will be responsible for completing page one of the form, the physician will complete the remaining pages.
- Please submit a copy of page five of the form as proof of exam completion. Upload the form via the CareDriver app.
