In some areas, a screening for tuberculosis is required to complete rides on the HopSkipDrive platform. TB screenings are currently required in:
- California
- Virginia
CareDrivers are responsible for any costs associated with the tuberculosis screening.

TB risk assessments vs. TB tests
Depending on your location, you may be required to take a risk assessment or a full TB skin test, blood test, or chest x-ray.
In California:
A Tuberculosis (TB) risk assessment is a questionnaire to assess TB risk that does not require a skin test. In California, a clear TB risk assessment is sufficient to move forward with certification. Download the California TB Risk Assessment form here.
If the risk assessment is not clear, you must take a TB test and obtain a negative test result to qualify to drive with HopSkipDrive. You may take a skin test, chest x-ray, or blood test.
In Virginia:
In Virginia, all CareDrivers must take a TB skin test or chest x-ray. A risk assessment is not sufficient. Learn more about how to obtain a TB Risk Assessment or TB skin test/chest x-ray below.
TB risk assessment
If you need to get a TB risk assessment, you can get one from your doctor or another medical provider.
If you have an existing clear TB risk assessment that is less than 60 days old, you can upload it using this form.
Required information for a TB Risk Assessment
An approved TB risk assessment must include the following:
- Full name
- Date of assessment and/or examination
- Date of birth
- Physician's signature, stamp, or office information
Only upload page 2 of the risk assessment form for review.
Option 1: Schedule an appointment with your doctor
Follow the steps below to receive a TB risk assessment from your doctor:
- Schedule an appointment with your doctor and ask them to perform a Tuberculosis risk assessment. Use the California Risk Assessment form here.
- Complete the risk assessment with your doctor. If you do not pass the risk assessment, you must complete a tuberculosis skin test, blood test, or chest x-ray, to qualify to drive with HopSkipDrive.
- Take a picture of page 2 of the clear risk assessment and submit it using this form.
Option 2: Other providers
You can also schedule a TB risk assessment with another medical provider, such as:
- CVS Minute Clinic
- WellnessMart
When you receive your results, take a picture of page 2 of the Risk Assessment form and upload it for review using this form.
Tuberculosis test
In some cases, you must pass a TB test. The TB test must be skin test or chest x-ray. In California, you may also submit a clear TB blood test. You must pass a TB test if:
- You are a CareDriver in Virginia
- Your TB risk assessment indicated a TB test was necessary.
If you have a current negative TB test, you can submit it using this form. HopSkipDrive considers TB tests current if they are less than two years old. While specific schools and organizations may have varying requirements (ranging from one to five years), HopSkipDrive enforces a two-year policy.
Required information for TB Test
An approved TB test must include the following:
- Full name
- Indication that a TB skin (PPD), blood (Quantiferon, CA only), or chest x-ray was completed
- Date of reading/test
- Physician's signature, stamp, or office information listed

- When you receive the results, take a picture of the results and submit them for review using this form.
