Yes! However, if you decline to provide information about your COVID-19 vaccination status, we will assume you are unvaccinated for purposes of matching you with ride opportunities that are different for fully COVID-19 vaccinated and unvaccinated persons.
What does “fully vaccinated” mean?
For purposes of this inquiry, an individual is considered “fully vaccinated” when it has been at least two weeks since receiving the final dose, as recommended by the manufacturer, of a vaccine that has been authorized by the FDA for use in the United States, including vaccinations that have been approved pursuant to an Emergency Use Authorization.
Should I send information directly to HopSkipDrive?

What happens if I provide false or inaccurate information?
Please note that you are required to provide accurate information about your COVID-19 vaccination status, or you may decline to provide your vaccination status. By submitting a response to the survey, you certify that you have accurately and truthfully answered the questions above. You also understand that if you falsify information related to this inquiry, you may be subject to removal from the HopSkipDrive Platform.
What happens if I choose an exemption?
If allowed by law or contractually, you can still access certain rides on the platform based on certain exemptions. For example, some markets allow religious and/or medical exemptions. While the specific requirements for these exemptions may vary across marketplaces, individuals may be able to claim an exemption based on acceptable and sincerely held religious beliefs or because of a risk for an adverse reaction related to an allergy to one of the vaccine components or a medical condition. In the event you choose a medical or religious exemption and additional information is needed, we will reach out to you.
What happens if I am not fully vaccinated yet but plan to get fully vaccinated?
If you indicate that you are partially vaccinated or plan to get fully vaccinated in your self-attestation, we will follow up with you before the deadline to provide a new attestation that meets the legal requirements in your area.
